Dr. Julie Keeler is a Civil Surgeon with over 10 years of experience conducting Immigration Physicals and completing i693 forms. Your physical and forms will be completed professionally and efficiently in her private practice in La Mesa dedicated solely to performing Immigration Physicals. No waiting in waiting rooms surrounded by sick people.
Important Information
What to bring to your appointment...

Government-issued photo identification, such as a valid passport or driver’s license

i693 forms with your portion completed

Copies of your vaccination records translated into English

Interpreter if you are not able to read, write and/or understand English with their portion of the i693 completed
(person on the forms MUST be present at the visit)

What happens at your appointment...

Dr Keeler will review your health history and perform a physical​

You will be referred to Quest Lab for blood tests (additional cost charged from lab)

You will be referred to your PCP, pharmacy like CVS or the county health department for any missing required vaccinations​​ (You will need to email documents proving you received the required vaccinations)
​​**If you have insurance, it will cover all blood tests and vaccines. If you don't, our patients receive a deep discount on blood test costs through Quest and vaccines can be obtained for free through the county health department with an order from Dr Keeler**

What happens after your appointment...​

Once your blood test results are received and you have emailed proof of any missing required vaccinations, your i693 will be completed by Dr Keeler

You will be contacted to arrange pick up of your completed i693 (original in a sealed envelope and copy for your personal records)

This whole process typically takes 5-7 days
